Individual: $35Family: $45
Student: $20 (with current undergraduate school ID)
Instructions:
1. For your convenience, the membership application form can be filled
out using Adobe Acrobat Reader and printed. (Note: You can NOT save or email
the form with Reader, only print it.)
2. If you prefer, you can print the blank form and complete it by hand.
3. The completed form and your dues check should be mailed to the address
shown on the form.
